Paraffin-embedded, formalin-fixed tissue block at 20-25°C.  Protect paraffin block from excessive heat.  Ship in cooled container during summer months. 
Unacceptable Conditions
Samples fixed in formalin substitute. 
Remarks
If paraffin block is unavailable to send, submit 12 unfixed tissue sections on "plus" charged or silanized slides.
